Despite efforts to advance telehealth as a care option before COVID, it progressed very slowly until the stresses of the pandemic forced rapid adoption and evolutionary change in the way we deliver care for chronic medical conditions. Providence used its telemedicine platforms in a variety of ways to address chronic illness, as patients were not comfortable coming in person for care. This led to a number of innovative programs for at home testing followed by virtual visits to check and treat chronic conditions. During this presentation we will discuss our overall approach, outcomes, lessons learned, and where we see telehealth evolving in the future.
